How to register for Online Services
For security reasons, we ask that you register in person at the surgery. It takes just a couple of minutes.
Reception staff will take a few details and ask you to complete our online services registration form, available at reception. You can also download and print the form here: Online Services Registration form
Registration can be done faster if you bring some identification and proof of address:
1) Photographic identification – passport, bus pass, or driving licence.
2) Proof of address – a recent utility bill or bank statement.
If you do not have any forms of identification, our reception staff can help you, and will ask you a few security questions in order to confirm your identity.

PATCHS online consultation service
PATCHS is different from the Airmid app, NHS app and SystmOnline. You can use PATCHS to submit routine medical problems or adminstration requests.
Please do not use PATCHS for prescription requests.
PATCHS online consulation service provides a convenient way for patients who don't need an urgent appointment to contact us at a time when it suits them. You can submit routine medical requests or adminsitrative requests 24 hours a day, 5 days a week. We aim to respond to requests within 72 hours and during surgery opening hours.
